CEU Program Application

The application must be submitted eight (8) weeks prior to the date of the program.

ALL documents must be submitted with this application.  Partial applications will not be accepted or reviewed.

Please note that until this application is approved, the availability of CEUs must be advertised on the program publicity as: CEU APPROVAL PENDING.
